  • The Financial Counselor will be responsible for establishing financial arrangements with the site office for patients. The Financial Counselor will also be responsible for identifying patient collection problems and assisting patients with billing inquiries.   • Triage daily patient phone calls in order of priority to ensure acute care needs are addressed by appropriate health care professional in a timely manner. Assist in contacting patients with lab/test results and schedule follow up as ordered by the provider when requested. Complete referral, shcool and sports forms.   • Provide leadership and clinical expertise for clinical staff to ensure quality patient care. Act as liaison between site managers, Doctors and clinical staff.     Three years of medical office experience including one year of supervisory level experience.

    Possession of a State License in nursing. CPR certification required (or ability to obtain certification within six months of employment.)

  • Coordinates care and manages high risk chronic patients from an assigned panel. Help develop and coordinate patient care programs and protocols focusing on those with serious, complex, or chronic health problems. Develop and coordinate patient outreach programs. Act as liaison between site managers, Doctors and clinical staff. Maintain proper education and in-servicing standards. Provide excellent customer service (friendly, courteous and helpful) to both our internal and external customers.

    Three years of clinical experience. Previous experience in care management, quality improvement, medical record reviews, disease management and/ or case management preferred. Basic level of skill with Microsoft Word, Excel and ability to use other computer programs and applications.

    Possession of a State License in nursing. CPR certification required (or ability to obtain certification within six months of employment.)
